Who We Are: Care & Prepare engages individuals, both healthy and those facing illness, in a proactive dialogue around Advance Care Planning to ensure that their wishes for end-of-life are clear and honored.
All adults over the age of 18 should have ACP education, as well as signed medical directives. Healthcare status can change in an instant. We have all seen catastrophic injury or illness sideline even the healthiest among us. If healthcare decisions haven't been made, this can lead to family conflict and unneeded anxiety at a time of already heightened stress. Our goal is to alleviate that anxiety by teaching others how to express their wishes and values when it comes to making important medical decisions that may affect their quality of life. In the Spring of 2021 our beta site started in Fort Worth & will someday grow statewide. Most of the audience we reach are elderly, on fixed income, & many terminally ill. Our pilot project "#ACPforALL" will target the elderly and Underserved communities because of their disproportional suffering & lack of ACP; to survey, inform them of their healthcare rights & responsibilities. Ideally, partnering with training institutions for research & to increase health literacy & health education of BIPOC & training new clinicians along the way.
In 2022 Care & Prepare served as a Project Talk site. #ProjectTalk initiative is a 5-year randomized controlled research initiative funded by the National Institutes of Health (NIH). The Project Talk Trial aims to engage underserved communities in advance care planning and empower them to start conversations about their end-of-life goals and wishes.
