Who We Are: Care & Prepare engages individuals, both healthy and those facing illness, in a proactive dialogue around Advance Care Planning to ensure that their wishes for end-of-life are clear and honored.
All adults over the age of 18 should have ACP education, as well as signed medical directives. Healthcare status can change in an instant. We have all seen catastrophic injury or illness sideline even the healthiest among us. If healthcare decisions haven't been made, this can lead to family conflict and unneeded anxiety at a time of already heightened stress. Our goal is to alleviate that anxiety by teaching others how to express their wishes and values when it comes to making important medical decisions that may affect their quality of life.
